Does Creatine Cause Hair Loss?

A single small study sparked the concern. Newer research that actually measured hair follicles tells a more reassuring story.

Does Creatine Cause Hair Loss?
Quick Answer

Probably not.

The idea that creatine causes hair loss largely traces back to a small 2009 study that reported an increase in dihydrotestosterone (DHT), a hormone involved in male-pattern hair loss in genetically susceptible people.

But that study did not measure hair loss itself, and its DHT finding was not consistently reproduced in later research.

More importantly, a 2025 randomized controlled trial directly measured both DHT and multiple markers of hair follicle health after 12 weeks of creatine supplementation. Researchers found no significant differences between creatine and placebo in DHT, hair density, hair count, follicular units, or hair thickness.

Current evidence therefore does not support the claim that standard-dose creatine causes hair loss.

What the Science Says

The creatine–hair loss concern began with a 2009 study involving college-aged male rugby players. After a seven-day creatine loading phase followed by two weeks of a maintenance dose, researchers observed an increase in circulating DHT. Because DHT contributes to androgenetic alopecia in genetically susceptible hair follicles, this result became the biological basis for the claim that creatine could accelerate baldness.

There were important limitations. Only 20 men entered the study, 16 completed it, hair loss was never measured, and the creatine group's DHT level was lower at baseline. Later reviews noted that the DHT finding had not been replicated and that most studies examining testosterone after creatine supplementation did not show meaningful increases in androgen levels.

The strongest direct evidence arrived in 2025. In a randomized, double-blind, placebo-controlled trial, resistance-trained men received either 5 g of creatine monohydrate per day or placebo for 12 weeks. Researchers assessed testosterone, DHT and objective hair measures using trichogram and digital imaging techniques. Thirty-eight participants completed the study.

There were no significant creatine-related changes in DHT or any measured hair outcome.

That does not prove that creatine could never influence hair loss in any individual, but it substantially weakens the original claim.

Why Did Creatine Get Linked to Hair Loss?

The concern began with DHT.

DHT is produced from testosterone through the enzyme 5-alpha-reductase. In people who are genetically susceptible to androgenetic alopecia, increased androgen signaling within certain scalp follicles contributes to progressive follicle miniaturization and thinner hair. Genetics plays a major role in determining who develops this condition.

In 2009, researchers reported that DHT rose after creatine supplementation in a small group of rugby players. The study quickly became the source of the idea that:

creatine → higher DHT → hair loss.

But the middle step does not automatically prove the final one.

The researchers did not measure hair density, hair shedding, follicle miniaturization, or baldness. And subsequent research did not establish a consistent creatine-induced rise in DHT.

What Did the New Hair Study Find?

The 2025 trial addressed the most important missing question: what actually happens to the hair?

Participants taking 5 g/day of creatine for 12 weeks showed no significant difference from placebo in:

  • DHT

  • DHT-to-testosterone ratio

  • total hair count

  • hair density

  • percentage of growing versus resting hairs

  • follicular unit count

  • terminal versus vellus hairs

  • cumulative hair thickness

This is considerably more relevant than simply measuring a hormone associated with hair loss.

Could Creatine Still Affect Someone Genetically Prone to Baldness?

We cannot completely rule that out.

Androgenetic alopecia is strongly influenced by genetics and follicular sensitivity to androgens not simply the concentration of DHT circulating in the blood. Recent genetic research describes the condition as a complex polygenic disorder involving hundreds of associated loci and multiple biological pathways.

The 2025 creatine study did not specifically select men with a strong family history of baldness, and family history was not formally assessed. The authors identified this as one of the study's limitations.

So the most accurate conclusion is not that creatine has been proven incapable of affecting hair in every conceivable circumstance.

It is that current human evidence does not show that standard-dose creatine causes or accelerates hair loss.

What If You Notice Hair Loss While Taking Creatine?

Timing alone does not establish causation.

Hair loss can occur for many reasons, including androgenetic alopecia, illness, significant weight loss, nutritional deficiencies, medications, hormonal changes and telogen effluvium. Hereditary pattern hair loss is also extremely common.

If hair loss is persistent, sudden, patchy, or accompanied by scalp symptoms, identifying the cause is more useful than automatically assuming creatine is responsible.

Limitations Summary

The evidence is reassuring but not definitive.

The first trial to directly evaluate creatine and hair health lasted only 12 weeks and included 38 resistance-trained men who completed the study. Women were not studied, family history of androgenetic alopecia was not formally assessed, and androgen concentrations were measured in blood rather than directly within scalp follicles.

The study also used 5 g/day of creatine without a loading phase, so it does not fully answer whether very high doses or different supplementation protocols could produce different effects.

Finally, hair loss particularly androgenetic alopecia develops over much longer periods than 12 weeks in many people.

Longer and larger trials would therefore strengthen confidence further.

What This Means in Real Life

If you take a standard daily dose of creatine and are worried that it will automatically make you go bald, current evidence should be reassuring.

There is no good evidence that creatine monohydrate at commonly used doses causes hair loss.

You also probably do not need to stop an otherwise useful creatine regimen solely because of the original 2009 DHT study.

However, if you are already experiencing noticeable hair thinning, especially with a strong family history of pattern hair loss, it makes sense to evaluate the hair loss on its own merits rather than trying to diagnose the cause from supplement timing alone.
